Please complete all 6 forms (see 'Forms' page above) and have your state-issued goverment ID available at the start of the examination. We reserve the right to cancel exam if these items are not present. Under no circumstances with Dr Alaniz begin an exam without verifying ID.
A clear scan of an ID (not a photograph of your ID) must be submitted to this office PRIOR to the start of the first exam, for those choosing an online appointment. NO EXCEPTIONS.
We require at least 24 hours' notice for cancellations or rescheduling of appointments. Failure to provide adequate notice may result in a fee. Due to the nature of concierge psychiatry, on rare occasions, we may require rescheduling on short notice (or conversion to a telemedicine appointment) if a patient crisis were to occur.
Other than the camera, these two services must be as equivalent as possible.
1) For online exams, patients are REQUIRED to have measured their own vital signs (on the day of the exam), which must be given to the doctor at the start of the exam. This includes weight, height, blood pressure, pulse, respiratory rate, and temperature. Failure to provide vitals may result in either termination of medications, cancellation of examination, and/or termination of treatment.
2) For online exams , the patient is required to have an area available in which privacy from others is possible. The doctor is not responsible if such an area is not available outside of the clinic (if the walls of your home or office are too thin, for example).
3) Laboratory testing is NOT OPTIONAL for routine medication management. All medications have a monitoring requirement (for example, CBC, CMP, lipid panel, TSH, etc.) that must be followed as a condition of treatment. These tests must be performed timely, either at the lab of your choice or in-office with Dr Alaniz. Recent lab reports from another doctor may be accepted as well. However, failure to comply with this requirement may result in termination of medications and/or termination of treatment.
Dr Alaniz does monitor physical health in addition to mental health, in order to screen for potential secondary causes or contributors to mental health problems (for example, thyroid dysfunction, uncontrolled diabetes, etc.).
